To attend the class, students must upload a screenshot of a signed waiver onto this form. This is an important step as it enables the instructor to know whether the waiver has been signed or not. Additionally, it helps to protect the instructor’s rights and interests. Failure to upload the waiver will result in the student being unable to attend the class.
