This form has two functions:

It’s important to provide the emergency contact details and blood type of all students who are attending the lesson so that in case of an emergency during the class, the instructor can send them to the hospital without any delay. This is essential for ensuring the safety of all students.
To attend the class, students must upload a screenshot of a signed waiver onto this form. This is an important step as it enables the instructor to know whether the waiver has been signed or not. Additionally, it helps to protect the instructor’s rights and interests. Failure to upload the waiver will result in the student being unable to attend the class.
